I presume 'it' is the TARDIS? The Doctor has made comments about it being impossible to cross back on a person's own time track (for example, he says that's why he can't simply take Donna to her wedding on time in The Runaway Bride. He's obviously not being completely true, as he and Rose crossed back on their own time tracks in Father's Day - but it may be the possibility of the Reapers turning up again is what prevents him. If the TARDIS was to turn into an empty shell again, he wouldn't be fixing it!

As for Susan... Again, the Doctor has said he knows he's the last Time Lord because he can't feel/sense any others in his head. The Master was, of course, turned to a human through a chameleon arch, so was unable to be found. Everyone else is dead, or otherwise not showing up as being a Time Lord.

(According to the novel Lungbarrow, Susan is from a time in Gallifrey's history before there were Time Lords, so she may not actually count in the terms of the Doctor's awareness of other Time Lords... The actual relationship between the Doctor and Susan both is and isn't one of grandfather and granddaughter in that novel. It's available free on the BBC Doctor Who websire, go read it if you want to know more!)
